These Terms of Service (“Terms”) govern your access to and use of VictoryMedicalAZ.com (the “Site”), operated by AZ Wellness Consultants LLC, d/b/a Victory Medical (“Victory Medical,” “we,” “us,” or “our”). By accessing or using the Site, submitting a form, subscribing to our newsletter, or opting in to SMS messaging, you agree to be bound by these Terms. If you do not agree, please do not use the Site.

The Site is intended to provide general information about Victory Medical’s services, allow visitors to request appointments or consultations, subscribe to our newsletter, and opt in to SMS communications. 2. No Medical Advice; No Doctor-Patient Relationship
The content on this Site, including any articles, descriptions of services, or general information, is provided for informational and educational purposes only. It is **not intended as, and should not be relied upon as, medical advice, diagnosis, or treatment**. Submitting an intake form, appointment request, or other inquiry through the Site does **not** create a doctor-patient (or provider-patient) relationship. A provider-patient relationship is established only after you have been formally accepted as a patient and have completed any required in-person or telehealth consultation and documentation processes.

3. Intake Forms and Appointment Requests
Forms on the Site allow you to submit information to request an appointment, consultation, or additional information about our services. Submission of a form does not guarantee that an appointment will be scheduled or that any particular service will be available to you. We reserve the right to accept or decline any prospective client/patient at our discretion, consistent with applicable law.
You agree to provide accurate, current, and complete information when submitting any form. Providing false information may result in delays, cancellation of your request, or refusal of service.

4. Email Newsletter and SMS Communications
**Email Newsletter.** By subscribing to our email newsletter, you consent to receive periodic emails from us containing special offers, event invitations, and informational articles. You may unsubscribe at any time as described in our Privacy Policy.
**SMS Messaging.** By providing your phone number and opting in through the applicable checkbox or form, you expressly consent to receive recurring marketing and informational text messages from AZ Wellness Consultants LLC d/b/a Victory Medical, including appointment reminders and promotional messages. Consent to receive SMS messages is not required as a condition of purchasing any goods or services.
– Reply **STOP** to any message to cancel your subscription at any time. You will receive a one-time confirmation message once you have been unsubscribed.
– Reply **HELP** to any message for assistance, or contact us using the information in Section 10 of our Privacy Policy.
– Message and data rates may apply. Message frequency varies.
These SMS terms are incorporated into, and governed by, both these Terms of Service and our Privacy Policy.
