Membership Service Options

DIRECT PRIMARY CARE

Includes:

  • All office visits related to ONLY general/non specialty primary care services
  • Text and email communication
  • 1 basic lab panel every 12 months
  • Same day or next day sick visits
  • Child membership for $70/month unless added to an active adult membership then the cost is $40/month
  • In office testing: EKG, urinalysis, HCG/pregnancy
  • Steroid joint injections
  • Trigger point injections
  • Injectable Medications: corticosteroid, anti-inflammatory (Toradol), antibiotic (Rocephin)
  • 10% off the following services:
    • a. Joint ADULT spouse/partner membership(s)
    • b. HRT pellets (bioidentical)
    • c. PRP injections
    • d. Tox/wrinkle relaxer injections
